IMPORTANT INFORMATION

VHSL Physical forms are required for all participation during the 2025-2026 school year and are good for 14 months from the date of the physician's signature. Completed forms may be turned in at the school's athletics office or emailed as a PDF file to mpearman@k12albemarle.org.
